There are two consent forms as part of this toolkit. Both can be adapted for your own use.

Here is a downloadable consent form for use by someone who has done some life story work to give permission to others to share parts of it, for example in research, a display or on this website. We would love to share more examples on this site to inspire people getting started and ask that you use this form if you or anyone you are working with would like to share images of their work.

 

The consent form below is for the storyteller to fill in at the beginning of doing Life Story work alongside a worker. 

First, go through and explain the information from the participants sheet.
